Collection Notice

Ramjan Health Services Pty Ltd ABN 75 634 732 308 (trading as Warragul Family Medicine (‘we’, ‘us’ or ‘our’) collects personal information about you in order to enable us to provide you with the best service experience possible on the website and for our internal business purposes that form part of normal business practices and for purposes otherwise set out in our Privacy Policy at https://www.warragulfamilymedicine.com.au/.

We collect this information directly from you, and in some cases from other general practitioners, specialist practitioners, or other health professionals involved in your care, always with appropriate consent.

We may disclose your personal information to third parties that help us deliver our services (including information technology suppliers, communication suppliers and our business partners) or as required by law. If you do not provide this information, we may not be able to provide all our services to you.

If you are providing information about a child, please ensure you are authorised to do so. We handle children’s personal information with additional care in line with our Privacy Policy.  By providing your personal information to us, you give your informed and voluntary consent to its collection, use and disclosure as described in this notice and in our Privacy Policy.
